Community Health Assessment and Community Health Improvement Plan

The Community Health Assessment (CHA) is a continuous process of collecting, analyzing, and sharing information about the health needs of our communities. GO Health collaborates with the Wyoming County Health Department, hospital systems, community organizations, and residents to assess health status, identify needs, address gaps, and recognize available resources. This process includes reviewing health data, engaging in discussions with individuals and groups, and conducting community surveys to understand public perceptions of health.

2022-2024 Community Health Improvement Plan

The Community Health Improvement Plan (CHIP) is a collaborative initiative across Genesee, Orleans, and Wyoming Counties, aimed at addressing health challenges based on community input and findings from the Community Health Assessment. It outlines evidence-based strategies and assesses efforts to track progress and measure outcomes.
