Community Health Assessment (CHA)
The Community Health Assessment (CHA) is an ongoing process of collecting, analyzing, and sharing information about the health needs of our communities.
GO Health collaborates with the Wyoming County Health Department, hospital systems, community organizations, and residents to assess health status, identify needs, address gaps, and recognize available resources. This includes reviewing health data, engaging with community members, and conducting surveys to better understand public health priorities.
