Services Available

Outpatient Clinic

  • Assessment – Completed by a therapist to determine the most appropriate services. This may take 1-3 sessions.
  • Therapy– You and your therapist will develop a treatment plan together to determine your therapy goals. Sessions will focus on working towards these goals using different therapy techniques. You may also receive psychoeducation on mental health topics relevant to you.
  • Medication Management– After discussion with your therapist, you may be referred to one of our psychiatrists or psychiatric nurse practitioners for a medication evaluation and follow up.
  • Referrals– To other agencies and services that can provide additional support to you.

Forensic Services

The Forensic Department services children and adults who are involved in the legal system. We work with individuals that are incarcerated as well as those being released from incarceration that are on parole or probation. We offer forensic evaluations for clients that are required to do so through court.

Care Management

Some individuals may qualify for care management services, which can assist with managing health needs and linkages to supports and services in the community. These connections may consist of linkages with primary care, physical health specialists, mental health and substance use professionals, social services, peer supports, educational supports, housing, medical assistance, budgeting, and other support services.

Care managers can assist with housing needs by making referrals to available programs but do not provide emergency housing.

Care management clients must meet certain eligibility criteria to receive services.  Criteria include certain medical and behavioral health diagnoses along with related risk factors.

S.P.O.A.- Single Point of Access:

SPOA is a process led by a SPOA Coordinator which helps Local Governmental Units achieve community-based mental health systems that are cohesive and well-coordinated in order to serve those individuals most in need of services. There are two types of SPOAs - Children and Adult. SPOAs assist with linkages to supportive services in the community, like Care Management and Adult Housing. The SPOA process identifies individuals most in need of services and manages service access and utilization.
