Children and Youth with Special Health Care Needs (CYSHCN)

This program provides information, referrals, and advocacy for children from birth to age 21 who may require health or related services beyond those typically needed. There are no eligibility requirements or fees to receive assistance.

Case Management services, funded by the Children and Youth with Special Health Care Needs Grant, offer outreach and referrals to connect families with appropriate services. Our goal is to help you support your child in reaching their full potential.

Available Assistance

• Accessing health insurance for your child
• Determining eligibility for Medicaid or Supplemental Security Income (SSI)
• Assistance obtaining medical services or equipment for home use
• Advocacy with service providers to ensure quality care
• Developing a support network for families
