Plan Ahead-Advance Directive

Advance Care Planning

New York State law provides several ways for caregivers to help manage the business, personal and medical care of a person with a disability.

Planning ahead is important. Talk with family members, caregivers and other trusted people about how you would want your health care handled if you were unable to make decisions for yourself.

These wishes should be documented through advance directives so others can understand and follow them. Advance directives may include:

  • a Health Care Proxy;
  • a Living Will; and
  • a Do Not Resuscitate (DNR) order.
